How to enter pin code in login screen when no keybord (only touch screen) is present

We have a smart card based windows login to our (kiosk like) pasient terminals. Each user gets a smart card, and are logged into windows XP by entering a key code.

The problem arises when we replace the keybord with a touch screen. Then we need to modify the windows XP login screen to include 10 big buttons (0-9) that the patiens can use to enter their key code.
Question is: How do we build a customized login screen with big (image)buttons for each number (0-9).

Yes, I know the tablet version has a build in on-screen-keybord, but this is not what we want. The keybord is to complecated, and not good looking.
