How to have titles (Mr., MS, etc.) displayed in an MS Word document...

When inserting a name, address, etc. from the ooutlook
address book, the title (such as Mr., Ms, Mrs, etc.) does
not display.

Example: Name in address book is Dr. John Smith
4545 Mockingbird Lane
Elkins, WV 26241

When I insert the information from the Outlook address
book, the Title is not displayed. All other information
appears.

Here is a response I received in microsoft.public.word.mailmerge.fields:

Hi Joe,
Using the Mail Merge Wizard, it doesn't seem possible to include the suffix
field from outlook into my document. Does anyone know a trick to get past
this?
Certain types of fields in Outlook aren't transferred if you set up the mail
merge from the Word side. Instead, go to Outlook, display the Contacts you
want to use in the merge, then use Outlook's Tools/Mail Merge command. This
way, you should get all the Outlook fields.
