How to I get Mail Merge to include Address 3 in a directory?

In an address block, I need title, company and 3 address lines. Mail merge
will not include the Address 3, even though I have matched the field.

Don't use the Address Block. Just insert the individual mergefields that
you wish use into the document in the configuration that you want them.
