HOW TO I WRITE OUT AN AFFIDAVIT?

I NEED TO WRITE OUT AN AFFIDAVIT FOR THE CHAMBER OF MAGISTRATE.
IM NOT SURE HOW TO SET IT OUT - DOES ANYONE HAVE ANY KNOWLEDGE?
