how to include header and footer in mailmerge?

when i mailmerge any document included a header an footer, the header and
footer messied.
how can i include them in my mail when i use mailmerge properity.
i now the document translated to another extintion, can we translated to a
picture so we can protect every thing in it eather heador or footer or any
thing else?

Note: i am using office 2003, outlook2003, word2003.
and window XP
