How to just have one page report instead of 36

Hi,

This is probably something very simple, but for the life of me, I can't
figure it out.

Thanks to the kind help from the dedicated people that reads my posts, I was
able to create a tally style report.

Now my problem is, I only need this to appear once, but it repeats itself
for every record I have. What do I need to do to just have it come up once?

Open the report with a WhereCondition that limits it to just the one record
you want.

The simplest way to do this is to use a form where you find the record you
want, and then click a button to print just that one reocrd. Here's how:
