How to list a name with the page number

Hello. My dad is an author - at 86 just published his first book. He is
getting ready to submit a second one to the publisher. They require that the
title of the book be at the top left of each page, and on the top right of
each page, they require his last name and page number: Estes-1, for example.
Is there a way to set this up automatically? He has been manually typing
these on each page. Then, of course, when he edits enough to add or subtract
lines from a page, he has to go back and retype these things over and over.

Header/footer. In the header on on the left side, type in the title and on
the right side type in his name and then click on Insert Page. Will
automatically number pages from then on.

For more help type Headers in help's search and then type in Page numbering.

If I had the version of Office (and I am assuming he is using Word to write
the book) I could be more specific on the header setup.

He can use Headers and Footers functions in Microsoft Word. You can access this
function by going to, VIEW, Headers and Footers

You can now toggle between headers and footers by clicking on the fourth icon
from last.

The headers/Footers can be at LEFT, CENTER, or RIGHT. Use your tab key to get
these.

I have assumed that he is using Microsoft Word to write his book.

Of course in my hurry to get this off I omitted the words Number Icon after
the statement "Insert Page" and this is in the Header and Footer toolbar in
Word 2003 and earlier.
