how to make a check box work

I have a fax cover sheet that has marco check box but I cant check or uncheck
the boxes. Please help

Im using office 2007 do you have any suggestions for that, cause I cant get

I've tried the method, and it appears to be working. The only problem is that
when I double click an unchecked box to make it become a checked box, it
moves everything following it on that line to the next line. Any suggestions
on how I can check a box and not have everything after it bumped to the next
line?

Also, I have no problem adding a check to a box, but I am unable to clear a
box once it is checked. Having trouble going back and forth between checked
and unchecked. Any advice?

I just checked to confirm that the toggle objects.dot (Pre-Word2007) and
toggle objects.dotm (Word 2007) template addins that are contained in the
toggle object.zip file on my website work as expected.

Did you build your toggle objects from scratch using the steps I provided?
The only thing I can think of that is causing your text to move down is that
you selected the paragraph mark when you created your checked box autotext.
