how to make a check box

I'm trying to make a form and want to put check-off boxes in several columns so user can choose options. I can make a box using "insert / symbols," but can't figure out how to keep it there when I go back to place an "X" or a checkmark.

Use the forms toolbar - view Toolbars Forms, and use the check box there.
