How to make a stacked column chart?

Can anyone tell me how to make a stacked column chart using rating data (eg, excellent=5, Very good = 4, etc)? I'm trying to enter data from a survey to chart and I am not sure how to enter the data from rating questions so that I can plot a stacked chart (rather than just average scores)

Mary -

You should put your data in this format:

Excellent Very Good Okay Fair Rotten
Ques 1
Ques 2
etc.

Keep the top left cell blank, and put either percentages, or the raw
counts, of each response in the grid below. Select the range, including
the top row, left column, and blank cell, start the chart wizard, and
create a stacked column chart, with series in columns.
