How to manipulate with headers/footers/page numbers

Hey I have a 3 page survey that I am mailing out to thousands of people for
work. We know exactly who these people are and have given each of them a
unique ID.

How do we play around with the page number/header/footer so that every copy
of the survey would have the ID corresponding to the specific client, and all
3 pages have the same ID on it.

Use mailmerge to create the documents from a data source that contains the
ID numbers and put a mergefield for that data in header where you want it to
appear.

If you are using Word XP or later, the “Add-in to Merge Letters to Separate
Files†that I have written and that can be downloaded from that site will
allow you to create each letter as a separate file with a filename taken
from a field in the data source with a minimum of fuss.

Using the information from those two sources, you could automate the
creation and emailing of the documents.
