How to repeat table format on all pages?

Hello

I am new to this, please excuse me if I wrote wrong here

I have a question on Microsft word 2007. I have created a table in wor
to document issues whenever a team member faces it. The table has cell
like Name, Date, Question on, What is the Question asked?, What is th
Answer recieved? etc and same number of empty cells to fill in th
details. My question is whenever someone creates new page, same forma
has to repeat. Is it possible to do that

I tried to use "Repeat Header rows" option in table toolbar but th
drawback of that is I cant edit the table. If I do anything to any cel
in the table it will be updated on all pages which makes me difficult t
keep different information on different pages.

Please help me in this
