How to retrieve data from html to excel?

Can any one help me the create html forum to retrieve data from webpage
to excel. currently i have three checkboxes to for the employees to
choose that is Snacks,Dinner and Snacks & dinner with the text area to
fill the comments when the submit button is clicked currently i have the
forum method to post the data to my mail id but i need the data in excel
spreadsheet.Please can anyone help?
