How to use clip art as choices in drop down menu?

I am trying to create an easy form for folks to use when processing
applications. The drop down menu would consists of
7 different pictures such as a car, a house, graduation cap. The user
would pick the picture from the images. Then have a text box that would be
next to the image in a different field to write in the personal information.
The form when Completed have the images in a list going down with
information next to them.

I am not sure if this is possible to do in info path or even as a template
in word 2003. HELP!!

Can a set group of pictures be used in Word, Info Path or Publisher in a drop
down menu? I don't need them to move or animate. If Not do you know of a
program that would be able to do this type of application? Thanks<JP
