hyperlinks within a document linked to headings don't work

I have a 56 page glossary. I have set all the names of definitions as
headings (heading 3). I have set hyperlinks to these headings throughout the
document. The hyperlinks now all link to the main title of the glossary
(Heading 1). If I edit a hyperlink is shows an address of the heading
starting with a # but the hyperlink does not take me there. What can I do?

If you answer the following question I (or someone else) may be able to help
you:

What version of Word are you using?
Is the glossary in the same document as the document containing the
hyperlinks?

If so, when the glossary was created, was it in the same document as the
document containing the hyperlinks?

Did you extensively edit (cut, paste, etc.) the glossary?

Have you entered manual page breaks before each glossary entry?

Describe precisely the steps you went through to create one of the
hyperlinks including how you entered the data in the Address box of the
Insert Hyperlink dialog box
