I am having problems with word not correctly indexing entries

Whilst most of the index appears as it should do there are various sections
where the software appears not to function for instance the 50 or so King
entries are listed individually with King and then the entry indented on the
next line.
Smith entries are nicely group until you reach Thomas where they all appear
in a long list !

It's not clear to me whether you are wanting the King entries to be grouped
or not, but if you *don't* want them grouped, make sure that the XE fields
contain no colons; if you do want them grouped, then make sure that each XE
field looks like { XE "King:Thomas" }, etc. That is, there must be a colon
between the main entry and the subentry.

I have checked the text and the entries all appear to have the appropriate
format.

I had this problem in a long document that I indexed. At the time, I just
gave up and corrected the problem after editing was complete (since I was
going to unlink the index and do some hand-editing on it, anyway), but in
retrospect, I suspect that the document was probably corrupt and might have
responded to some of the treatments described at
http://word.mvps.org/FAQs/AppErrors/CorruptDoc.htm
