I can not get submit button work on a web form

I am trying to generate a Form, using word, that will allow a user to go to a
URL, answer some questions and click submit. Then Questoins & answers will
come back to an email address so we can save this data in Word or even
Notepad.
When I click the SUBMIT button, the only information I get is:
Submit=Submit Query

Can anybody tell me what all I am doing wrong?
