I can not view my custom form in all public folders

my custom form is in task folder in all public folders, but I can't see it
after creating new item or editing older items.
New items couldn't be saved: The item could not be saved. The folder may
have been deleted or moved. Or, you may not have appropriate permisions for
this folder.....

I configure this folder for using this custom form only. If I configure
folder for using any form creating of new items is successful.
This custom form is fully working in another folder.

What is wrong on my form?
