This sometimes happens with the Switchboard Manager and once the Table
"SwitchBoard Items" (?) exists, Access assumes the Form "Switchboard Form"
exists and won't create a new one.
If you haven't done much work with the items, simply delete the Table
"Switchboard Items", simply delete this Table and run the Switchboard
Manager again to create the new Form / new Table.
If you have done a bit of work on the items and want to retain these items,
do the following:
* Rename the Table "Switchboard Items" to something else so that the
Switchboard Manager doesn't "see" it.
* Run the Switchboard Manager again and the SM will create the new Form /
new Table.
* Delete the new Table and rename the old Table back to the correct name
"Switchboard Items" (?).
The new Form will pick up the existing items in this case.
