I need a geneaology (family tree) template or form.

I am looking for a form that can be completed with names, dates of birth,
death, children, marriages, etc., called a geneaology or family tree form. I
am compiling my family's history and would like something in Word on which I
can insert the information for each individual and their immediate family.
Does anyone have or know of such a form that I may use?

It’s possible that the MS-hosted bank of templates by MS and third
parties will offer something:

But you would be better off buying dedicated family tree software.
Google for some recommendations. Word will not be your friend
here--you're likely to spend more time fighting it than recording your
family's history.
