I need help creating a database/template!!!

Hello! I have been assigned the task of creating a database for logging
periods of time "unavailable" for the section of the call center where I am
employed. I need the data base to be able to be filled in by the employees
and approved or disapproved by management after they submit the information.
If anyone has any direction or insight they can give, I'd be extremely
appreciative. I'd need input sections titled: Date, Manager, Employee ID
(alpha-numeric), First name, Last name, Start Time, End Time, Comments and
then a separate section for manager use only, titled Approve/Disapprove/
