I need student questionnaire 10 Qs + comments + multi-choice?

I need a simple questionnaire of 10 questions that will return to a useful
report providing results from comment boxes, yes/no answers and multiple
choice answers. Anyone have a template for this please?

The best tool for the job might not be Excel. I suggest using Survey Monkey
or a similar service. If the service's charts aren't what you want, you
could download the data results and use Excel's Pivot Tables for analyses
and charts.
