I need the database notations for visio creating an ERN

I am trying to do an ERN in Visio and can't find the notations I need for
cardinality and just need to get them so I can associate them or is it that I
create the entity and create the notations by how I define the entity?

Try going to Database | Options | Document. Select the Relationship tab and
check Cardinality.
