I need to place in Outlook company procedures.

I need to create in Outlook, a form that let me to capture the company
procedures.
Could you please help me?
How can I do it adding my logo?
Each department will be capturing its own procedure so, the date of the last
modification must be showed.
Also I need a format in blank ready to be use for new procedures.
The whole procedure should be published in Outlook and in that way the
current information must be there.
