Icons in custom forms

I created a custom form in Outlook 2003 (beta) and selected a new .ico
file for the icon. It the properties section of the custom form the
icon shows up for both the big and small version, but when I base a
contact off of that form, the icon does not show-it defaults back to
the ID card icon. Any ideas anybody?

Did you select different icon images for the small and large icons?
