Icons - tearing my hair out

I have tried everything I can think of.

I have a custom form with a custom icon. The form is based on IPM.Post and I
have published it to a Public Folder.

Yesterday I had several items based on the form saved in the public folder
in question, and they obediently displayed my custom icon. Then I did
something, not sure what, and they all jumped to showing a closed envelope.

The form has been one-offed? Well I thought so at first, but the message
class of the items is still the custom form, and the code behind the form
still runs, so I don't think they can be one-offed.

Maybe my form cache is corrupted? Well I found and renamed frmscache.dat,
but the icons won't come back, so I don't think its that.

I've run out of ideas. Anyone have any more?
