Identifying and changing Avery template numbers

I just printed some previously designed business cards on Avery stock and they didn't line up correctly. I may be using the wrong Avery stock.
1. How do I see which Avery template number was used when the form was designed?
2. How do I change the Avery template number after the form is designed?
