identifying Form names within Public Folders

We have 6000 Public folders and 34 Public Forms. We are
trying to quantify how many of the Public Forms are
currently being used and which Public folders are storing
the forms. We currently have a tool that tells us things
like folder size and number of items, etc. But
unfortunately doesn't tell us which form created the
item. Also, We have the ability to modify the tool but
we don't know which call to use to tell us which form
created the item. Does anyone know of a way to do this?

The MessageClass property contains the name of the form.
