IE keeps asking for certificate

regarding www.gasolinealleyantiques.com
problem with secure order page: https://www.gasolinealleyantiques.com/orderssl1.htm

Internet Explorer had pop up window whenever customer tried to click
onto secure order page asking for CERTIFICATE. had tech people at
hosting company "require client certificates" turned OFF on IIS.
worked EXCEPT now when you click the SUBMIT button to go to the
confirmation page, the pop up is appearing again.

Website tech people have no clue what to do. checked with the Thawte
certificate people and they insist it is a server problem.

I am a simple businessman acting as Webmaster for his own site. I am
at wit's end. If I did not explain accurately - as me what you need
to know. anybody out there have a clue? help greatly appreciated.

Haven't tried your form
- but see 2 problems with it
1) Do not name form fields with reserved characters
(like s&h or s&h - use s-h instead)
2) You are saving all form results to a txt files on the server (_private/aaaorderssl.txt)
- That is forbidden by all credit card companies and is not secure
(you need to send the cc results to a secure credit card processing service and never store them on your site)

You also have a JavaScript error on the page with SymRealWinOpen
