If statement in a custom form

Using Office XP - I have been asked to create a survey
form in Outlook. In this survey they want to be able to
skip some questions depending on how a question is
answered. Like if on question "1" they check "No", then
goto question "4" else goto question "2". Does anyone
know where there is a code sample for this type of
scenario, or other information that might send me in the
right direction?
