1) I've never used the default folder under wwwroot for my pages ... I
always create new webs using File>New Web, and enter the name. When browsing
the local machine you just add the web to the end, ie
http://localhost/myweb/, ensuring the default file name for the opening file
is as expected under IIS config (eg default.htm). These webs you create ...
ie myweb ... are subwebs in relation to the wwwroot web. You can have as
many as you want ... I've got 20 or so for different projects on the go.
2) My IIS version is 5.1. My server extensions (at least under properties
for /LM/W3SVC/1 I assume this is the SE version number) are 4.0.2.5322. I
suspect this version is reported because I don't have XP SPs on this
machine --- I'm behind a hardware firewall and virus-protected.

There is something definitely whacky with your setup, and it may have to do
with service packs you have that I don't have installed. Perhaps others can
chime in here if there are issues, or if they have further suggestions
concerning your error.
If not, there is the second last resort ... following a backup in case
things go awry, I would do the following in an attempt to resolve the
problem:
a) open Administrative Programs > Internet Information Services, and
right-click on 'default web site', ... there you should have an option under
Tasks to 'Remove server extensions". Do so.
b) uninstall FrontPage
c) through control panes>add/remove programs/windows, uninstall IIS.
d) reboot
e) ensure all folders directories under C:\Program Files\microsoft FrontPage
are empty, and if not, delete it. Also delete the entire inetpub folder --
this will delete your local ftp and wwwroot folder and all contents.
f) delete the FrontPage temp folders under your temp folder(s). I have 2
.... one under c:\temp, where my environment variable points as the tmp
location, and one under c:\windows\temp.
g) delete C:\Documents and Settings\birchr\Application
Data\Microsoft\FrontPage
h) rename the folder C:\WINDOWS\system32\inetsrv if it still exists
i) delete C:\WINDOWS\inf\iis.inf and C:\WINDOWS\inf\iis.pnf if it still
exists
j) delete or rename C:\WINDOWS\system32\adsiis.dll , adsiisex.dll,
iisext.dll , iismap.dll, iismui.dll , iisreset.exe , iisrstap.dll ,
iisRtl.dll , iissuba.dll, and spiisupd.exe if they still exist
k) do a registry search for 'FrontPage and examine any results found to
determine if they appear 'harmless' to a subsequent reinstall.
l) reboot ... there should be no startup errors. If there are note them and
try to resolve the error by inspecting the registry. There should be no IIS
entry under administrative tools, nor a Server extension entry. In addition,
if you open mmc and go to file>add/remove snap in>add the IIS and FP server
extensions should not be listed.
m) get a coffee
n) install IIS from add/remove programs/windows. Reboot.
o) install FrontPage
p) open fp ... go to file > new web and try to create a dummy web site of
any name. If you can and you can add and open a page, you're fixed. If not,
get a second coffee, or perhaps a scotch.
I've only had one machine (a win2000 box) that did not "take" the server
extensions. I tried to debug the problems for about a half-hour, but as I
had installed those immediately following an install of Windows 2000,
instead of dicking around further I decided that nuking the drive,
reformatting and reinstalling Windows and IIS again would be better in the
long run. Although a PITA, at least in this instance no data needed to be
preserved.
My last resort suggestion would be to do this, especially if you have no
problem with XP/IIS on other machines.
As I said earlier, others may have additional hints specific to service
packs you may have installed and the error message the extensions are
generating.
