I've got the exact same problem.
Not sure but I think it started after I installed and uninstalled Acrobat
Writer.
Could be a coincident.

I use two computers - Laptop and Desktop on same net.
Laptop is Ok - Desktop has problem.

Application name Outlook 2003 SP3
Version 11.0
Build 8169

Looking for Help - IT here is stumped - I don't want to reload System.

1. Locate the Outlook Temporary Items folder by opening the Registry and
locating HKEY_CURRENT_USER\Software\Microsoft\Office\11.0\Outlook\Security\
OutlookSecureTempFolder
OR if you don’t want to walk a user through the Registry, open an attachment
in any email, do a “SAVE AS†and it will default to the path for the OLK***
folder.
2. Navigate to the value of this Key. It should be something like:
%USERPROFILE%\Local Settings\Temporary Internet Files\OLK3D\ where OLK3D is
some randomly generated string that always starts with OLK
3. Since OLK*** is a hidden folder, you can navigate as far as “Temporary
Internet Files†and then add OLK*** to the end of the path and press enter.
4. Quit Outlook
5. Locate the file. (You may have to search for it) Delete the contents of
the folder
6. Launch Outlook

We have done a little different approach if you delete the registry key
"HKEY_CURRENT_USER\Software\Microsoft\Office\11.0\Outlook\Security\OutlookSecureTempFolder
it works for awhile then the red x appears again. If I go to the path of the
value of the reg key the OLK### folder is not there and I have allowed view
hidden files and folders.

Why does it keep reoccuring and why is the OLK folder not there

The folder is there. WIndows Explorer will not show it, even with viewing
hidden files and folders enabled.

Got it okay I took the path and paste, delete contents now lets see if the
issue reoccurs. If it does reoccur is there a permanet fix ?

Microsoft has been trying to fix the problem of the Secure Temp folder for a
long time. Apparently there are so many conditions whereby it can be left
populated that MS can't find them all.

I've read all this and information from other sources. I have a customer
having reported the same problem yesterday. I will try to use the fixes I
found today on his computer later. However, if this thing is a local problem,
how come that when he sends a mail with an empty placeholder to me. I see the
same as he does. The problem seems propagated. When I use that mail to
forward or reply, the picture becomes visible like magic. When i takes his
mail send to me in his sent items box and forwards it, it also becomes
suddenly visible. We both work on windows XP and Outlook 2003 fully updated.
Other times he manages to create a new message with the picture visible too.
It's almost like an on-off situation. Once you get the signature picture,
once you don't, on his machine and on machines that receives his emails.
