IMAP messages downloaded every day

I'm using Entourage 2004 on Mac OS 10.4.11 (I'd love to upgrade to
2008, but I can't because of the lack of support for Palm OS
devices). I use it with a Fastmail IMAP account.

Every time I start Entourage, I find that none of my messages (or even
headers) are downloaded to my computer, although it does have an
accurate count of new messages. So I have to select whichever folder
I want, and then wait for all of the messages to be downloaded (which
can be in the thousands).

Is there a setting I'm missing?
I've checked off both "Always download complete message bodies" and
"Partially receive messages over 20 KB". I've also checked off "Live
sync" but not "Connect only to inbox", and I've also checked off
"Check for unread messages in subscribed folders".

Could the problem be related to my setting for root folder? I seem to
remember that it started after I changed this setting, but I don't
remember what it was before. Now it's "INBOX", which is what they
recommend, although it might have been ".INBOX" (with the preceding
period) before. They do say in their recommended settings that a
period should be the "folder separator", but I don't see a setting for
that in Entourage.

Also, another problem which might be caused by this is that I have a
custom mail view which sort of resets every time I start Entourage
anew. All filter criteria that refer to specific Fastmail folders
reset to the inbox.

Lastly, Entourage now often takes a while to start up. Don't know if
it's related to any of the preceding.

Is there a setting I'm missing?

Under Tools --> Schedules, select the Send & Receive All schedule. Click on
Account Options and check the folder you want. See example about half way
down this page:
