Import Contacts from Lotus Notes Address Book

I can't import my Lotus Notes contacts (Address Book) into Outlook.
Whether I save the Lotus addresses as a vcard or as a csv file, either way, when I try to import it into Outlook, it only appears to import one contact, and that one is garbage anyway.

Any ideas?

I can look at the file that Lotus outputs (at least the .csv file), and it looks okay to my eyes. Here is a representation of the first two lines. Any ideas.
