Import Word Option Buttons into Access

I'm trying to import information from a word template into an Access
database. I'm having a problem writing code to import the option buttons.
This document is a survey which has 5 options per statement. I've grouped
the 5 options by naming them the same name under Group Name. This is working
so only one option can be selected. I don't know how to retrieve these
options and import the result into the Access Database.

I need the lines for entering the box. I also have check boxes and can't
seem to import those in correctly either.
I know it should say something like: !s21 = replace..... but can't get the
info correct.

Thanks but I have read those articles the other day. They do not match my
question.

I'd appreciate any other help you can provide.
