Importing contact data from sql-source to user defined contact form

Hallo,

i´m looking for an possibility to import contact data from an
sql-source (csv and xls-format also available) to an user defined
outlook contact form.

I´ve just found a way to import in the standard form but not for user
defined.

Thank you for all tipps.
