Importing Contacts

I am trying to import some contact from an Access database to Outlook and
have run into a couple of problems:

1) The import wizard doesn't off me all of the available fields when
mapping fields from my csv file to Outlook. How can I import without using
the wizard or have the wizard offer me all of the fields.

2) In the Access database, there is a boolean field that I would like to
capture in Outlook. How can I get that field mapped to an existing field?
If that can't be done, can I create a field and map to that?
