Importing from Lotus Organizer 5.0 to Outlook 2000

Can anyone help with importing a lotus 5.0 file into
Outlook 2000 ? I followed exactly the procedure described
in the Microsoft Knowledge Base Article - 257890
OL2000: "How to Import Lotus Organizer 6.0 Data".
However, after mapping custom fields as described in step
9, I can't get to click "OK" as in step 10, because
the "OK" button is still grayed out.
Any suggestions are appreciated, as I have a lot of
appointments I want to switch entirely to Outlook 2000.
