in FP 2002 - What is wrong with my code

I am not a programer.

Using front page 2002 I built a simple page to have visitors request an
E-letter that I send to my clients. Here is the link

I need to have the registration info e-mailed to
(e-mail address removed).

But it is not working. Can you please tell me what is wrong in my code.

You need to contact the author of the email script (winqcgi.pl) you are using to process the form
data.

This was generated using a contact management software that I use called
Maximizer. The company not provide much support.

Do you know where I can find a simple HTHL code that I can insert into a
page creating a form to collected this data e-mailed to
(e-mail address removed).

If your site is hosted on a web server that has the FP extensions (you can open the site via http or
upload to it via http), then you can use the default FP Form handler.
