Inconsistency in Outllook, forms.

I was wondering if anyone can help me with the following
problem. We use Outlook "Contacts" as our Database.
We have templeted a form with more data fields than
the 'normal' fields. However this form does not always
come up, (sometimes just the plain one)when we try to
choose it.
Further confusion is caused because sometimes it brings
up the correct form and sometimes the incorrect one. On
the same day.
The only clue is its says, "Loading form" prior to the
times it works correctly.
Can anyone help?

It might be helpful if you mentioned your Outlook version and where the form
is published.

A rough guess from the information you've provided(and without knowing which
version of Outlook you are using) would indicate that it's likely to be
something to do with the forms cache - read up on this on MSDN.
