Index Dividers

I am trying to find a template for the index page of a package of 15 tab
dividers. The instructions that are included for making my own template are
for Word 6.0 and just don't seem to work for me. I have triple checked my
entries but it's still going onto two pages. I've tried the Avery web site
but the one they have there doesn't line up either. This seems to happen
quite often so any suggestions would be greatly appreciated.

You may have to create your own if the template doedsn't work. I'd start
with that template tho and tweak it until it did work.
