Index does not show in document ...

Create index have marked all entries using a cncordance file. Display field
codes and the fields beginning with {XE ... } are all there, but the index
itself is not generated. Now it is a BIG word doc over 1 million characters,
and is 1,166 pages long.
Any idea about what I can do to solve this problem?

Have you gone to the end of the document and inserted an INDEX field?
(Insert | Reference | Index and Tables | Index)
