Indexing errors

When indexing a document, marking the items to be indexed inserts additional
text that results in the pages being misnumbered (identified in the index
print out.) How can this be avoided?

Is it necessary to manually mark each index entry for inclusion in the index
printout? Titles are in Bold while all other text is not. Is auto indexing
available in this situation?

Create a concordance of the terms to be indexed and use auto indexing.
