Indexing formats are inconsistent

Using the command to create index entries, when the fields are updated,
formatting is all over the map.
Some entries take the formatting (and any formatting changes made to the
style itself) while others revert to the font of the body text when F9 is
pressed.

Any reason for this?

The reason is they are fields. If you use the autoindex capability it
generally doesn't create as much of a problem. Also, if you are
manually indexing, make certain to be consistent in applying the index
style. Hope that helps.
