I found this great link for my fatal attempt to create Input Parameters for a
form using Jscript.

however, since I don't know Jscript very well (ok more like at all) I'm
having a hard time making the code work. Can I ask for a simple example for
Jscript that takes in the input parameter and assigns it to a textbox?

The fatal attempt I did make gives me an error of:

The form contains XML that cannot be parsed:
An invalid character was found in text content.
