InfoPath 2007, SharePoint 2007 and a workflow for a Request Form

Hi All. Here is the scenario, and then I will write my question/needs.

Scenario:

A VPN Access Request Form has been created in InfoPath 2007 and published to
MOSS 2007. It has 2 verifactions in it: an "I Agree" Box for the requestor,
and then it needs a second verification for the Supervisor of the requestor.

The Problem:
I cannot figure out a way to make it so that the Supervisor (via a workflow)
is notified that he/she has a request to approve. Due to the library being a
Form Library, I cannot set permissions to make things secure from other
users. Due to hierarchy issues within the organization, I also cannot create
a user group for Supervisors. In order to fill out the form, all users must
have Contributor access - so technically, anyone can approve/reject a request.

I am desperately looking for help, advice, answers, anything that will move
me along a bit more.

To rephrase the requirement, there is a checkboz in the form which a
requestor. When the requestor checks this checkbox and submits the
form, the supervisor of this requestor should be intimated that there
is a request pending for approval.

If this is the case, can you let me know as to how supervisor and
requestor are linked with each other?

Hi Paresh -

That is correct. Best case scenario would be:

Once the box is checked and submit is clicked, the form then goes into a
SharePointlibrary. The next step is where I am stuck.

Now i need the Supervisor to be notified, presumedly via workflow and email,
and then the supervisor will go to the library, verify the information is
correct and then approve the form.

My biggest issue is security - because it is a Form Library, I am VERY
limited in how I can secure the forms so that a supervisor is the only one
who can approve a request form. Since you must have at least contributor
rights to fill out a form, you automatically have the ability to
approve/reject a form.

Also, i cannot make a group for supervisors since this is a constantly
changing roster.

Any help is GREATLY appreciated.
