Ink annotations versus drawing canvas

When using ink annotations, what is the best way
to "lock" the inked writing such that it does not end up
moving around when one adds any further typed text above
or near the ink? This is a problem that makes adding ink
to a document (such as a simple signature or a checkmark)
problematic.

The alternative is to use ink drawing (i.e. the drawing
canvas) to add some inked portions to notes. However,
this seems cumbersome to me. For example, it is
difficult to write on the bottom of a page and then
continue handwriting on the next page; the entire
drawing canvas jumps to the second page if your
handwriting does not fit at the bottom of the first.

My goal is to have my medical notes a combination of type
as well as ink. I originally set up typed templates, but
then want to ink in comments, checkmarks, notes, and my
actual signature using ink. Seems like the ideal way to
do medical charts in a natural way using a Tablet, but
thus far I haven't found the easiest way of implementing
this.
