Insert vs Update

When InfoPath does a submit, how does it know whether to do an Insert or an
Update? I would like to be able to programmatically do this. Is there a
flag that is set to distinguish between the 2? Does the "Insert Item" option
set off a flag? If so, what is the name of the flag, so that I can use it,
since I'm not using the "Insert Item" option? Using VB.Net.
