Inserting image in header/footer

I need to insert company logo into form's header so it would show up on
every page when the form is printed.

Is that posssible?

And one more thing, infopath places wrong "page width guide" when the page
is set to A4, also, print preview is all messed up. To demostrate te
problem

None of the elements are "sticking out" of the form, I've chacked that. The
same problems manifests itself when there's only a simple 3 column table.

You cannot directly insert image in the header, instead you have to insert
it as a background image. Go through this link
