Integrating with Active directory/misc form question

I am new to this forum and here is what im trying to accomplish. I have an
AD domain environment with Office 2003 and Windows XP. I would like users to
fill out an electronic form when they need IT resources. For example, a user
needs access to the Sales share on the network. User fills out an electronic
form choosing the share name from a combo box and then the appropriate share
owner (sales manager in this case) is notified and approves/denies the
request. After the share owner approves the req, the IT admin (me) gets an
email and I act accordingly and "close" the req and both share owner and user
are notified. Is that something I can do with Forms server or infopath or do
i need both, etc? I will continue scouring the MS site for more info. Thanks
